Kaye has been a Registered Nurse for 16 years. She has a hands on experience working with elderly in a Skilled Nursing Facility in Hawaii., she worked in an Assisted Living Facility and RCFE as well. With Kaye's dedication to excellence, she was chosen to be part of the most prestigious hospital in Hawaii, Straub Pacific Health, HI. When not caring for her patients and case managing discharges from acute settings, you will din Kaye in craft fairs, volunteer work and traveling with her friends and family.

Ryan has served the elderly community in their most vulnerable times. He has worked as Social Services assistant, responsible in the day to day pyschosocial needs of his patients in a Skilled Nursing Facility. He has honed his active listening skills when residents are in distress and has an in depth knowledge of a safe and complete discharge planning.

Ryan ensures that he listens to the families request, considers the patient's current level of care and tour the family until they are fully satisfied to what fits to their standard. Ryan ensures a seamless and flawless transition of the patient from Acute to RCFE.

Michelle has been in health care for more than 12 years. From a receptionist of a skilled nursing facility, to Admissions Coordinator, to Social Services to becoming a Marketing Director. Michelle's heart is always at the right place to help the geriatric community.

When not at work, Michelle enjoys spending quality time with her children.
